A comparison of the effects of two triphasic oral contraceptives on haemostasis.

The effects of two cyclically administered, triphasic, combined low dosage oestrogen and progestogen oral contraceptives on haemostasis have been compared in a longitudinal study, over 6 months, in 26 healthy females aged 16-30 years. Subjects received either Logynon (ethinyl oestradiol and Levonorgestrol, n = 14) or SHD 415G (Schering U.K., n = 12), which contains a similar dosage of ethinyl oestradiol, but in combination with a new progestogen, gestodene. Both groups showed increases in biological activities of procoagulant factors fibrinogen, X and XII and decreased activity of the naturally occurring coagulation inhibitor antithrombin III (AT-III). The majority of these changes were statistically significant (P less than 0.05 to less than 0.001), apparent after one cycle and maintained over the six cycle period. FVII activity increased in both groups, achieving statistical significance (P less than 0.01) by cycle 6 in the SHD 415G group but not in the females receiving Logynon. Protein C activity remained unchanged in both groups. Between-group comparisons showed no differences in the procoagulant factor changes, but protein C was lower (P less than 0.05) in the SHD 415G group after three cycles of therapy. These findings indicate that both triphasic oral contraceptives Logynon and SHD 415G induce increases in procoagulant factor activities which are not balanced by increased biological levels of the two most important physiological coagulation inhibitors AT-III and protein C. These prothrombotic changes are not modified by the new progestogen, gestodene.

Clinicopharmacological examination of Anteovin.

PIP: The effectiveness, side effects and serum FSH, LH, prolactin, estradiol and progesterone levels were monitored in 79 women taking Anteovin for a total of 506 cycles (mean 6.4 cycles). Anteovin is a biphasic oral contraceptive with 11 tablets containing 0.05 mg levonorgestrel and 0.05 mg ethinyl estradiol, and 10 tablets containing 0.125 mg levonorgestrel and 0.05 mg ethinyl estradiol. There were no pregnancies. 11 women dropped out because of hepatomegaly (1), bleeding disorder (3), gastric pain and nausea (1), breast pain (1), nausea and vomiting (1), and personal reasons (4). 10.3% of those continuing reported minor side effects. Menses occurred every 28 days for 3-5 days, with 2 cases of breakthrough bleeding but no oversuppression. Progesterone ranged from 1.5-2.0 nmol/1, estradiol varied between 68-93 pmol/1, LH stayed constant at 6.3-11 U/1, FSH remained at 5.8-6.8 U-1 without a peak, and prolactin levels were lower than those seen in control cycles. These hormone levels all resemble those observed in anovulatory cycles. This pill is especially suitable for teens and nulliparas.

Comparative data on the liver- and lipid metabolism influencing effects of Anteovin.

PIP: Data on blood lipids and liver function from 90 young women from a computer file on oral contraceptive users were analyzed. Subjects selected were 30 Bisecurin users, 30 Ovidon users, and 30 Anteovin users, aged 17-25, who had smoked 20 cigarettes/day or less. Triglycerides increased significantly (p0.001) at 1, 3 and 6 months for all groups. Total cholesterol also rose significantly for all groups at all time points. HDL rose significantly in Bisecurin and Anteovin users, and fell in the Ovidon group, at 6 months, and variable at other times. The liver enzymes GGT and GPT increased progressively, while AP fell. Laboratory values remained within normal limits. Varicose veins in 1 patient was the only complication. Side effects were listed with each pill formulation; Anteovin caused fewer side effects than the other pills.
