Very-low-calorie diet in the treatment of massive obesity: preliminary experience.

Twenty-two consecutive obese patients were given very-low-calorie diet (VLCD) in the form of a casein and lactose preparation dissolved in orange juice containing 43.0 g protein, 30.4 g carbohydrate and 1.1 g lipid in a total of 317 kcal (1.3 MJ) daily. VLCD was given intermittently in eight-week periods alternating with three-week periods of conventional diet (900-1200 kcal, 3.8-5.0 MJ) to ensure recuperation of lean body mass and to establish a therapy which may facilitate indefinite patient compliance. In case of persisting hunger, diethyl-propion was given in small doses. The median observation time was nine weeks (range 1-37 weeks). The median weight loss was 15.5 kg (range 3.8-39.2 kg), and the median overweight decreased from 85.5 per cent (range 43-135 per cent) to 61.0 per cent (range 19-100 per cent). Blood glucose and cholesterol fell significantly (P < 0.01), and no major side-effects were observed.

Influence of pH on hydrolytic decomposition of diethylpropion hydrochloride: stability studies on drug substance and tablets using high-performance liquid chromatography.

The influence of pH on the hydrolysis of diethylpropion hydrochloride was investigated. Hydrolytic decomposition of the drug in solution at 45 degrees occurred at a very slow and constant rate at pH 3.5 and below but increased rapidly as the pH was raised above 3.5. Results of stability studies performed on six commercial tablet products from five manufacturers showed a similar relationship between the decomposition rate and the formulation pH. A reaction pathway based on the formation of an enamine structure is proposed for the pH-dependent hydrolysis. A high-performance liquid chromatographic assay for the drug is presented, which also separates and detects the hydrolysis product 1-phenyl-1,2-propanedione and the compound assumed to be the enol tautomer of this product.

Controlled-release diethylpropion hydrochloride used in a program for weight reduction.

In a double-blind, placebo-controlled evaluation in obese adults given comparable dietary and exercise recommendations, controlled-release diethylpropion hydrochloride promoted significantly more weight loss than matching placebo. The mean reduction in 12 weeks was 15.9 lb (average 1.32 lb/week) for 12 patients taking diethylpropion hydrochloride, 10.0 lb (0.84 lb/week) for 13 taking placebo, and 12.2 lb for 13 taking drug for two four-week periods separated by four weeks of placebo (1.38 lb/week on active drug and 0.30 lb/week on placebo). Amphetamine-like side effects were virtually absent. Diethylpropion hydrochloride is an effective adjunct to caloric restriction in therapy of obesity.
