Lansoprazole and its derivative AG-1789 dose-dependently inhibited cellular respiration by an endogenous substrate and decreased the ATP level in Helicobacter pylori cells. The inhibitory action of lansoprazole and AG-1789 against respiration was specific to substrates such as pyruvate and alpha-ketoglutarate and similar to the inhibitory action of rotenone, which is an inhibitor for the mitochondrial respiratory chain. Growth inhibition by lansoprazole and AG-1789 as well as by rotenone was augmented at high oxygen concentrations under atmospheric conditions. Since the 50% inhibitory concentrations of these compounds for the respiration were close to their MICs for H. pylori growth, the growth inhibition might be due to respiratory inhibition by these compounds.

Lansoprazole, a new substituted benzimidazole, is an effective acid proton pump inhibitor acting by inhibiting selectively H+/K+ ATPase of the gastric parietal cell. This study was performed to assess the effect of successive 30, 60, 90 and 120 mg dosages of lansoprazole in 4 patients suffering from Zollinger-Ellison syndrome. The basal gastric acid output was markedly inhibited in comparison with baseline values (mean maximal reduction: 87%; extremes: 75-99%) and was dose-related. Lansoprazole inhibited pepsin output globally with a dose range effect between 30 and 90 mg/day. The treatment induced a rapid relief of clinical symptoms. No biological abnormality was noted. These data proved that lansoprazole is efficient for treating gastric acid hypersecretion in patients suffering from ZES.

Bactericidal activity of lansoprazole and omeprazole against Helicobacter pylori in vitro.

The minimum inhibitory concentrations of 39 Helicobacter pylori strains against antibiotics (ampicillin, tetracycline, metronidazole), colloidal bismuth subcitrate and proton pump inhibitors were assessed by the agar dilution method. 20.5% of the strains were metronidazole resistent. The comparison of the bactericidal action of omeprazole and lansoprazole in a killing curve assay showed a dose-dependent reduction of viable bacteria after 24 h, lansoprazole being more effective than omeprazole. Under varying pH conditions, the antibacterial activity of lansoprazole was limited to a pH range of 6-7, reducing bacterial numbers up to 4 logarithmical steps within 24 h.
