Concentrations of phenoxymethylpenicillin and cefadroxil in tonsillar tissue and tonsillar surface fluid.

Thirty patients who underwent elective tonsillectomy were given phenoxymethylpenicillin (0.8 g) or cefadroxil (1 g) at different times before operation. The concentrations of the antibiotics were analysed in serum, tonsillar tissue, fluid from the surface of the tonsils, and mixed saliva. The concentrations in tonsillar tissue for both drugs were much lower than the corresponding serum concentrations. This apparently low tissue accessibility could be ascribed to the limited intracellular penetration of beta-lactam antibiotics. For both antibiotics the concentrations in the tonsillar surface fluid were higher than the levels in the tissue and well above the minimal inhibitory concentrations for streptococci. This was not due to antibiotics in saliva but probably a result of leakage from the interstitial fluid. Inability to reach active concentrations of phenoxymethylpenicillin or cefadroxil at the site of infection does not therefore seem to be a probable cause for relapse after treatment of streptococcal tonsillitis.

Colonization of human genital tract by Streptococcus agalactiae.

This bacteriological study involved 92 young married couples who required medical help because of infertility. Colonization of male urethra or the presence of Streptococcus agalactiae in the ejaculate was detected in 9.78% of men. Vagina or vagina and cervix were colonized in 4.35% of women. Oral application of phenoxymethylpenicillin temporarily eliminated S. agalactiae from the genital tract of all colonized partners. Repeated colonization occurred 3-7 months later, in three cases by different serotypes compared with pretreatment period, in one case by the same serotype.

Cefuroxime axetil and penicillin V compared in the treatment of group A beta-hemolytic streptococcal pharyngitis.

Patients with the signs and symptoms of acute tonsillopharyngitis were treated with cefuroxime axetil, an orally administered, beta-lactamase stable cephalosporin, or penicillin V for ten days. Group A beta-hemolytic streptococcal (GABHS) infection was confirmed bacteriologically in 115 patients. Patients aged 13 to 18 years received 250 mg of cefuroxime or 500 mg of penicillin V twice daily. Bacteriologic cure was found in 33 (94%) of 35 patients treated with the cefuroxime and in 12 (67%) of 18 treated with penicillin (P less than 0.05). Patients aged 4 to 12 years who received 125 mg of cefuroxime axetil twice daily also experienced a greater rate of bacteriologic cure than patients who received 250 mg of penicillin V three times daily, but the difference was not statistically significant. Cefuroxime axetil is at least as effective as penicillin V in the management of streptococcal pharyngitis and may be more effective in preventing the carrier state.
