Plasmid carriage and antibiotic susceptibility of Neisseria meningitidis strains isolated in Sweden 1981-1990.

A random selection of Neisseria meningitidis strains isolated in Sweden in the period 1981-1990 were investigated for plasmid carriage and susceptibility to antimicrobial agents commonly used for treatment and prophylaxis of meningococcal disease. The MICs were determined by the agar dilution method for penicillin V, penicillin G, rifampicin, sulfadiazine, erythromycin and tetracycline. In 13% of the invasive strains the MIC of penicillin V was > or = 0.5 mg/l which may cause concern regarding the usefulness of penicillin V in prophylaxis. In strains isolated from the urogenital tract the MICs of penicillin V and penicillin G were higher than in the invasive strains. In about 82% of the strains isolated in 1987-1988 the MIC of tetracycline was > or = 0.5 mg/l whereas no such strains were found in 1981-1982. Plasmids were found in 2 of 119 invasive strains, in 1 of 50 strains from the respiratory tract and in 1 of 19 strains from the urogenital tract. The plasmid sizes were 1.3, 2.6, 25 and 40 Mda. None of these strains were beta-lactamase producing and no relation to a high degree antibiotic resistance was observed.

Incidence, technique of isolation, and treatment of group B streptococci in obstetric patients.

A prospective study was performed to determine the incidence of group B streptococci cultured from the cervix in a series of obstetric clinic patients at St. Margaret's Hospital. Cultures were taken during each trimester and planted to both a blood agar plate and a selective broth medium. Of a total of 812 cultures, 18 were positive, an over-all incidence of 2.2 per cent. During the first, second, and third trimesters, 2.2 per cent, 2.5 per cent, and 1.9 per cent of cultures were positive, respectively. Women with positive cultures were treated with a 10 day course of oral penicillin or a suitable alternative; four of 16 had persistent positive cultures at the end of the first treatment period, three of these four responded to a second course of antibiotics, and the remaining patient responded after a third treatment course. The total number of positive cultures by selective broth was 24, compared to nine in blood agar, indicating a 2.7-fold increase in pickup by the selective broth.

Further observations on the value of oral penicillins in chronic staphylococcal osteomyelitis.

This report confirms after periods of observation of up to nine years, the successful treatment with high oral doses of cloxacillin or phenoxymethylpenicillin of 19 patients suffering from chronic staphylococcal osteomyelits. The antibiotic treatment was closely supervised over long periods, and the satisfactory response initially reported has been maintained in each case. Brief reference is made also to the use of the same treatment in a further 191 patients with chronic staphylococcal bone infection. Observations of the progress of 136 of these patients are continuing. At this stage the response in these patients confirms the value in chronic osteomyelitis of the long-term treatment with these penicillins which was established in the first 19 patients treated.
