Evaluation of tricyclic antidepressant plasma levels by an automated enzyme immunoassay (EMIT) in comparison to a high-performance liquid chromatographic method.

A new homogeneous enzyme immunoassay technique (EMIT) for the measurement of plasma levels of amitriptyline, nortriptyline, imipramine, and desipramine was used with an automated procedure and the results were compared to those of a high-performance liquid chromatographic (HPLC) method. Precision of the EMIT test was similar to that of the HPLC method with within-day coefficients of variation in the range of 3.9-10.9% (EMIT) and 3.9-9.6% (HPLC). The day-to-day coefficients of variation ranged from 4.4 to 11.7% for EMIT and from 6.1 to 8.4% for HPLC. Samples from 124 patients were analyzed by both methods and a good correlation was observed for all the four drugs. A paired t test indicated no significant difference for the EMIT and HPLC values. No significant interferences were observed between the tricyclics tested and other commonly associated drugs, such as benzodiazepines and neuroleptics. The new EMIT assay proved to be rapid and easy to perform and showed sufficient reliability and reproducibility to be used for either emergency or routine analysis.

Determination of compliance with riboflavin in an antidepressive therapy.

In a comprehensive study on the fate of amitriptyline (At, Laroxyl) in depressive patients, it was investigated whether their compliance could be controlled in analysing the concentration of riboflavin (vitamin B2) in urine. Therefore, they were medicated for 3 weeks with special tablets (Ro 4-1575/063) containing 150 mg At and 15 mg riboflavin. Different modalities or urine samplings were designed in order to establish the optimal conditions for testing compliance. Riboflavin, creatinine, At and nortriptyline were measured in urine, the tricyclic drugs also in plasma. The results show that compliance could be tested in analysing urine before and 4 h after the intake of the drug. However, this design was associated with a considerable subjective constraint for the patient.

Interaction of perphenazine with the kinetics of nortriptyline.

The kinetics of nortriptyline in four patients were investigated before and during treatment with perphenazine (24-36 mg/day) by administering 57 mg nortriptyline hydrochloride as an intravenous infusion and evaluating the resulting plasma concentration data according to a two compartment open model. In all patients an increased biological half-life as well as a decreased systemic clearance and rate of metabolism were found in the perphenazine-period, thus confirming the inhibitory effect of perphenazine on the metabolism of nortriptyline found in earlier studies. Some interaction with distribution parameters was also indicated, but on the whole the model parameters did not provide much further information about the interaction. Although the most pronounced changes were found with the patient getting the highest dose of perphenazine, the dose-effect relationship remains obscure.
