[Gemfibrozil in the treatment of hyperlipoproteinemias]

30 patients with primary hyperlipoproteinemia (type II and IV according to Fredrickson) were treated with Gemfibrosil (Gevinon--film tablets of 450 mg of "Parke-Davis Company") for a period of 45 days with a daily dose of two tablets taken with the evening meal. The results of the treatment were a decrease of the total cholesterol (with 16.34%), of triglycerides (with 34.50%), of low density lipoproteins (with 10.76%) and of very low density lipoproteins (with 49.21%) and an increase of the high density lipoproteins (with 2.34%). Gevilon was well tolerated. No side effects requiring discontinuing of the treatment were registered.

Within the framework of clinical tests of the preparation Gevilon (gemfibrosil tablets 450 mg) of Parke-Davis Co. this hypolipidaemic preparation was administered to 28 patients with different types of hyperlipoproteinaemia. One-month administration of gemfibrosil, 900 mg per day, significantly influenced some parameters of the lipid and lipoprotein metabolism. Plasma cholesterol declined by 20%, triglycerides by as much as 60%. On the other hand, the HDL cholesterol level did not change. The concentration of the "risk" apolipoprotein B declined by 11%, that of apolipoprotein A1 which is considered protective from the aspect of ischaemic heart disease increased by 21%. There is a significant decline of lipoprotein Lp(a) which was not described in hypolipidaemic drugs of the clofibrate type. Treatment with Gevilon led also to a marked decline of the serum uric acid level. Gemfibrisol is according to the authors' experience as well as according to the results of other authors an effective hypolipidaemic agent suitable for the majority of patients with hyperlipoproteinaemia. Treatment with gemfibrosil leads to a significant decline of the prevalence of ischaemic heart disease.

An elevated serum cholesterol level is a well known major risk factor for developing atherosclerosis in general, and for coronary heart disease in particular. There are many lipid lowering agents currently available. We used gemfibrozil in twenty hyperlipidemic cases who failed to response to diet control for three months. They were thirteen males and seven females with their ages ranging from thirty to eighty-one year-old. They included ten diabetes, one nephrotic syndrome and nine pure hyperlipidemic patients. All cases received gemfibrozil 600mg twice daily for 4-12 weeks. Gemfibrozil caused an increase in HDL cholesterol. The reductions in serum level of total cholesterol, total cholesterol/HDL cholesterol, and triglyceride were found. Only one case developed mild gastrointestinal side effect. There was no other major side effects.
