[Changes in plasma volume and electrolytes after treatment of therapy-resistant hypertension with diazoxide and furosemide]

10 patients (5 females, 5 males, average age 50.2 +/- 5.8 years) with a therapy-resistant hypertension were for 10 days treated with injections of diazoxide and furosemide (hypertonalum and furesis). A significant decrease of the blood pressure as well as a decrease of sodium and potassium in the serum occurred, where as the electrolyte content of te 24-hour collection urine was influenced in homogeneously. The blood and plasma volume, respectively, was also significantly reduced. Heart rate and blood sugar significantly increased, urea-N, creatinine, hematocrit and hemoglobin did not increase significantly. The new regulation of the blood pressure to an oral antihypertensive permanent therapy continued 4 weeks to 6 months.

Relationship between kallikrein and PRA after intravenous furosemide.

Interrelationships between plasma renin activity (PRA), urinary kallikrein and sodium excretion were studied before and after furosemide iv administration in nine normal volunteers and in one low renin non hypertensive patient. PRA, urinary kallikrein and sodium excretion increased within 15 min of furosemide injection in nine subjects; kallikrein excretion then decreased sharply, whereas plasma renin activity reached peak values within 15-120 min of stimulation. In low renin subject low basal levels of PRA paralled undetectable values of kallikrein excretion, and PRA and kallikrein excretion showed no increase after furosemide, despite the expected natriuretic response. The following conclusions appear feasible: (i) the natriuretic effect of fuorsemide is direct and independent of the effect on PRA and urinary kallikrein; (ii) in man, furosemide induces an increase in urinary kallikrein excretion which is immediate, of short duration and simultaneous in all the patient studied; (iii) no statistical correlation is demonstrated between the temporal behavior of urinary kallikrein and PRA; it is however possible that, at least for certain stimuli, renin release is in some way correlated with the activation of the renal kallikrein-bradykinin system.

Relationship of urinary kallikrein excretion to urinary potassium excretion during furosemide administration with and without amiloride.

The relationship of urinary kallikrein excretion to urine volume, and to urinary sodium and potassium excretions was studied in normal rats during furosemide diuresis and superimposed injection of amiloride, a K+-sparing diuretic. Continuous infusion of furosemide increased urinary kallikrein, sodium and potassium excretions and the urine volume. Amiloride injection during furosemide diuresis caused further increase in diuresis and natriuresis, but a prompt decrease in urinary kallikrein excretion to basal level, and potassium excretion to below the basal level. The significant correlation of urinary kallikrein excretion to urinary potassium excretion, but not to urine volume and urinary sodium excretion after amiloride injection suggests that the major determinant of urinary kallikrein excretion is renal potassium secretion through a mechanism that is affected by amiloride.
