Impact of NSAIDS on murine antigen induced arthritis. I. Investigation of antiinflammatory and chondroprotective effects.

The well known nonsteroidal antiinflammatory drugs (NSAID) diclofenac, piroxicam and tiaprofenic acid as well as the corticosteroid prednisolone, were examined for their effect on injury to knee joint structures evoked by a developing murine antigen induced arthritis. To ensure the human therapeutic dose, each drug was given twice a day per os in 3 different dosage regimes. The severity of arthritis was monitored with 99mTc-uptake measurements. After 3 weeks, mice were sacrificed and knee joints were examined histologically. All drugs exhibited, in comparable extent, antiinflammatory effects in terms of swelling. Grading of joint damage was focused on cartilage harm and chondrocyte activity. Only prednisolone provided sufficient protection and seemed to stimulate repair activity. The NSAID had no clear beneficial or adverse influences on affected cartilage. Apparently, under these experimental circumstances, only prednisolone functions chondroprotectively.

Phototoxicity of non-steroidal anti-inflammatory drugs demonstrated in vitro by a photo-basophil-histamine-release test.

The phototoxic activity of the non-steroidal anti-inflammatory drugs (NSAID) acetylsalicylic acid, benoxaprofen, carprofen, diclofenac, indoprofen, ketoprofen, tiaprofenic acid, and of thiophene, a heterocyclic ring structure of the tiaprofenic acid molecule, was evaluated in vitro by a newly developed photo-basophil-histamine-release test (PBHRT) performed with human leukocytes. Cell suspensions incubated with 10(-6) to 10(-3) M of the test compounds were exposed to 1 to 100 J/cm2 UVA. Maximum photo-basophil-histamine-release was 4% with acetylsalicylic acid, 10% with benoxaprofen, 20% with thiophene, 28% with diclofenac, 39% with tiaprofenic acid, 40% with carprofen, 55% with ketoprofen, and not demonstrable with indoprofen. These results indicate that many NSAID can exert phototoxic effects, thus confirming clinical observations as well as other in vitro experiments. The PBHRT seems to be a promising new method for the identification of phototoxic compounds.

Influence of two non-steroidal anti-inflammatory drugs on lipolysis and on plasma post-heparin lipoprotein lipase activity in normal man.

Indomethacin 50 mg i.v. or p.o. and diclofenac sodium 50 mg p.o. produced a prompt and significant increase in plasma free fatty acid concentration. In 10 subjects who took indomethacin 150 mg/d p.o. for 3 days, plasma post-heparin lipoprotein lipase activity was also significantly increased. The same effect occurred in 9 subjects treated for 3 days with diclofenac sodium 50 mg t.d.s. Since both indomethacin and diclofenac sodium are potent inhibitors of prostaglandin synthetase, these findings are consistent with the hypothesis tht prostaglandins are involved in the feed-back regulation of lipolysis, and mediate the inhibitory effect of lipolysis on lipoprotein lipase activity.
