These recommendations update information regarding the polysaccharide vaccine licensed in the United States for use against disease caused by Neisseria meningitidis serogroups A, C, Y, and W-135, as well as antimicrobial agents for chemoprophylaxis against meningococcal disease (superseding MMWR 1985;34:255-9). This report provides additional information regarding meningococcal vaccines and the addition of ciprofloxacin and ceftriaxone as acceptable alternatives to rifampin for chemoprophylaxis in selected populations.

[Local treatment of otitis media and otitis externa: the role of quinolones]

Patients suffering from suppurative otitis media and externa, and in whom pseudomonas species are present, were treated twice daily by local instillation of ciprofloxacin 0.2%. Treatment resulted in clinical and bacteriological cure in all 17 patients, including patients resistant to previous therapy, within a mean time of six days. There were no adverse effects except one case of local allergy. Audiograms remained unchanged. Local therapy with ciprofloxacin is a welcome addition but should be reserved for cases resistant to other therapy.

Relationship between outer membrane protein profiles and resistance to ceftazidime, imipenem, and ciprofloxacin in Pseudomonas aeruginosa isolates from bacteremic patients.

Outer membrane protein (OMP) profiles of 122 Pseudomonas aeruginosa isolates recovered from the blood of bacteremic patients were analyzed to relate alterations in the expression of OMPs with porin activity to resistance to imipenem, ceftazidime, and ciprofloxacin. Imipenem-resistant isolates lacked or expressed reduced amounts of porin OprD. In contrast, alterations of OMP profiles were absent in most ceftazidime-resistant isolates. Six of 12 ciprofloxacin-resistant isolates had normal OMP profiles. The remaining isolates showed alterations in the expression of either OprC, OprF, or OprD. In addition, imipenem- and ceftazidime-resistant isolates displayed a beta-lactamase activity compatible with that of a group 1 chromosomal cephalosporinase.
