Neurosurgery without shaving: indications and results.

The objective of the investigation was to prevent the stigmatizing effect of a totally or partially shaved head with openly visible signs of a head operation, easing the reintegration of the patient into his daily life. After extended surface cleaning with a colourless, regular skin disinfection liquid (undyed isopropanol/dibrom/propylenglycol solution) the hair was combed apart from the incision line before draping. Wound closure was performed as usual, taking care to remove meticulously hair from the wound. To aid closure we made use of a 20% chlorhexidine jelly that holds the hair away from the incision. A neomycin ointment served for sealing the wound surface, no further dressing being used. After 215 cranial neurosurgical operations, among them 63 for tumours, 33 stereotactic procedures 18 shunt, 55 for torticollis and 46 other operations performed without shaving the hair, we saw one wound infection (0.5%). This percentage corresponds very well to our general infection rate of 0.6%. All patients very much appreciated the offered opportunity and the result. If the objective is to give patients a psychological advantage by preventing a partially bald head we think one can safely refrain from the shaving procedure without risking a higher infection rate.

[Good results with Norethisterone in rheumatoid arthritis]

PIP: The incidence of rheumatoid arthritis in women is 4 times that in men, suggesting a possible relationship between this disease and ovarian functioning. A study was done to attempt ovarian control in the treatment of rheumatoid polyarthritis by the use of norethisterone in a dose of 20 mg/day for 1 month. 3 women between the ages of 40 and 50 were helped greatly. In 5 women between the ages of 50 and 60 results obtained were quite good. 4 women over 60 had poor results. It is logical to suppose that results would be better in younger women. In 4 cases where metrorrhagia supervened and in 3 others treatment was not properly used. Some patients complained of loss of hair and in some there was a tendency to obesity and hypertension which, however, was generally mild. No case of rheumatoid arthritis was cured. Response, when it occurred, was rapid but once obtained was not followed by further improvement.

OBJECTIVE: To set up an inner ear ischemic model in guinea pig with ferromagnetic embolism. METHODS: A magnet was fitted in the external auditory canal and carbonyl iron filings (1%, 1 ml/kg) was injected into jugular, then the inner ear vessels were obstructed by ferromagnetic spheres. Cochlear blood flow (CBF) and number of red blood cells in the stria vessels were used to detect the model's ischemia of cochleae. The slice of temple bone and basal membrane stained by silver nitrate were used for inner ear's histopathological observations. RESULTS: The iron spheres were amassed in the one and two-day-later's model of inner ear vessels, which resulted in embolism. The number of red blood cells in the stria vessels decreased and then recovered to normal level after 4 days, but the CBF decreased to 50% +/- 10% of basic level immediately and recovered to 99% +/- 41% 4 days later. Scattered lesion of out hair cell cilium could be seen in cochleae in eight-day-later's model, and degenerations in different degree were found in vascular stria. CONCLUSION: The methods of inner ear ischemic model with ferromagnetic embolism could be practical and the decrease of CBF was reversible, so it may be an ideal model for studying some ischemic inner ear diseases and evaluating the effects of therapeutic drugs.

The hair growth promoting effect of Sophora flavescens extract and its molecular regulation.

In search of natural extracts for hair growth, we found that the extract of dried root of Sophora flavescens has outstanding hair growth promoting effect. After topical application of Sophora flavescens extract onto the back of C57BL/6 mice, the earlier conversion of telogen-to-anagen was induced. The growth of dermal papilla cells cultured in vitro, however, was not affected by Sophora flavescens extract treatment. RT-PCR analysis showed that Sophora flavescens extract induced mRNA levels of growth factors such as IGF-1 and KGF in dermal papilla cells, suggesting that the effects of Sophora flavescens extract on hair growth may be mediated through the regulation of growth factors in dermal papilla cells. In addition, the Sophora flavescens extract revealed to possess potent inhibitory effect on the type II 5alpha-reductase activity. Taken together, these results suggest that Sophora flavescens extract has hair growth promoting potential and can be used for hair growing products.
