Menstrual changes during Norplant use and return of fertility following Norplant implant removal: the Singapore experience.

Singh K, Viegas OA, Ratnam SS.

PIP: A evaluation study of NORPLANT in Singapore found the implants to be a highly effective, safe, acceptable, and reversible contraceptive method. the subdermal placing of 6 silastic capsules (3.4 cm in length and 2.4 mm in diameter) each containing 36 mg of levonorgestrel. In 1985, 100 women were recruited to receive the implants. The women had to be between 18-40 years of age, sexually active, of demonstrate fertility, neither pregnant nor breastfeeding, not have used any long-acting steroidal contraception 6 months prior to the insertion, and not have any of the standard contraindications involved in the use of steroids. In 1989, the data from clinical visits was examined. The data showed no accidental pregnancies during the period. 35 of the women did request the removal of the capsules, thereby terminating the treatment. 21 of these women wanted to become pregnant, 14 of whom had expressed a desire for more children at the time of insertion. Of the remaining women who opted for removal, 12 had complained of menstrual disturbances, 1 had weight loss of 7 kg over a period of 8 months, and 1 had complained of hair loss during the 3rd year of use. Following removal, the menstrual patterns of those suffering from disturbances returned to normal, and the hair loss subsided for the woman suffering from hair los (1 year after removal, though, the woman who had complained of weight loss had yet to increase weight). Concerning the women hoping to have another child, 81% of them became pregnant within 12 months of removal, a rate comparable to fecundity rates of former IUD and Depo-provera users. the study concludes that NORPLANT is an acceptable contraceptive method which has the potential for wide use in the future.

Uncommon cytidine-homopolymer dimorphism in 5'-UTR of the human otoferlin gene.

Human otoferlin, homologous to the Caenorhabditis elegans spermatogenesis factor FER-1 that was shown to be involved in membrane vesicle fusion, belongs to a group of membrane-anchored cytosolic proteins and is found expressed in brain, cochlear inner hair cells and vestibular type I sensory cells. Nonsense and missense mutations of OTOF lead to an autosomal recessive deafness phenotype (DFNB9). We describe here an unusual C-homopolymer dimorphism at position -136 of 5'-UTR of the OTOF short splice form. Although at first identified within a family with a hereditary component of hearing deficiency this C3/C5 dimorphism is found frequently in European populations (0.4 for C3, 0.6 for C5) and does not segregate with the deafness phenotype. The polymorphic site may become useful for studying the origin of different OTOF mutations within various populations, for assessing recombination events within large pedigrees as well as founder effects and for association studies in further deafness phenotypes.

OBJECTIVE: To determine prevalence of adverse effects associated with epidural administration of morphine with or without bupivacaine in dogs and cats undergoing surgery and evaluate effects of epidural administration of morphine on postoperative pain severity. DESIGN: Retrospective study. ANIMALS: 242 dogs and 23 cats. PROCEDURE: Morphine with or without bupivacaine was administered prior to surgery with a Tuohy needle, spinal needle, or epidural catheter. In 18 dogs that underwent surgery twice, results of preemptive epidural administration of morphine with or without bupivacaine were compared with results of systemic administration of oxymorphone and ketoprofen. RESULTS: The delivered fraction of isoflurane was significantly lower in animals given morphine and bupivacaine than in animals given morphine alone. Analgesia was of significantly longer duration in dogs given morphine and bupivacaine than in dogs given morphine alone. During anesthesia, mild respiratory and cardiovascular depression was reported. Seven dogs and 2 cats had urine retention, and 2 dogs developed pruritus. Six dogs vomited when a second dose of morphine was given epidurally the day after surgery. Eight of 72 dogs had delayed hair growth. In 18 dogs that underwent surgery twice, the delivered fraction of isoflurane was significantly lower and the duration of analgesia was significantly longer when morphine with or without bupivacaine was given epidurally than when oxymorphone and ketoprofen were given. CONCLUSIONS AND CLINICAL RELEVANCE: Results suggest that preemptive epidural administration of morphine with or without bupivacaine is a safe and effective method of inducing long-lasting analgesia in dogs and cats and is superior to standard management of postoperative pain with repeated injection of oxymorphone and ketoprofen.

Backgr Notes Ser. 1989 Feb;:1-5.
Malawi.

United States. Department of State. Bureau of Public Affairs.

PIP: Malawi is located in southeastern Africa with Mozambique, Tanzania and Zambia as its neighboring countries. The country's climate is subtropical, conducive to the tobacco, tea, coffee and sugar grown there. The population of Malawi is predominantly rural with 90% of the population engaging in subsistence farming. Most of the people are of Bantu origin. There is no visa requirement for those travelling in Malawi, but there are conditions to be aware of for the visitor to Malawi. The government bans the wearing of slacks or shorts by women and hair below the nape of the neck for men. Malaria is endemic and tsetse flies and the risk of encephalitis are present. The water, however, is potable and telecommunications within Malawi are good. The political structure of the country is headed by an elected present and a presidentially appointed cabinet. There are 118 seats in the country's unicameral National Assembly. Although Malawi has traditionally been self-sufficient, it suffered from the world commodity price recession in the 1980s. However, Malawi has continued to grow economically at a slower rate. The United States and Malawi enjoy good diplomatic relations and have an open exchange program. As well, relations with neighboring countries, particularly Mozambique, have improved greatly.

Hair loss is a problem in modern soceity. Examining the factors of hair growth may shed light on how hair loss might occur. How long can hair grow before it stops growing eventually if it does? Given that the hair growth rate is quite uniform and constant, somewhere between 0.3-0.5 millimeters per day, it's believed that the length of anagen, the growth phase, differs among individuals, and this is the major determinant to the maximum hair length. For some individuals, anagen may last ten years. Of course the length of the anagen is governed by genes, and the genetic background of the individuals. Non-genetic factors such as nutritional condition, weather, seasonal changes (hair may grow a bit faster during winter), taking medications, health condition may of course influence the rate of hair growth as well as The shape of the hair, straight or curly, is dependent on the shape of the follicle. A circular or round hair follicle would generate straight hair, while the follicle with oval or elliptical shapes (in its cross-section) would produce a curly hair.

DHEA has been suggested to provide numerous potential benefits. DHEA (or dehydroepiandrosterone) is converted into androgens (male hormones) or estrogens (female hormones) in the cells.
