Nonscarring hair loss disorders: the basis for recognition and treatment.

Hair loss can be assessed objectively by examining a hair pull sample for stage of growth and hair shaft diameter, getting an estimate of average daily hair loss, and mapping scalp hair density. The history of the loss, the clinical picture, and the growth stage and diameter of the hair when lost all help to establish the type of loss. Hair regrowth occurs without treatment in most types of hair loss. There is as yet no effective treatment for pattern or senescent alopecia, although in women the loss can now be retarded somewhat.

Round window membrane permeability to transforming growth factor-alpha: an in vitro study.

OBJECTIVE: Recent studies have identified the capacity of transforming growth factor-alpha (TGF-alpha) to stimulate mammalian labyrinthine hair cell regeneration after acute ototoxic damage. Augmenting hair cell regeneration with such growth factors may have a role in potentiation of recovery of cochlear function after hair cell injury. Transtympanic application of aqueous solutions to the round window membrane (RWM) has proved successful as a drug delivery route. The purpose of this study was to test the permeability of the mammalian RWM to TGF-alpha in an inexpensive and reliable in vitro model. METHODS: Guinea pig RWM niches were harvested and transferred to a 2-chamber apparatus, and TGF-alpha was applied to the middle-ear side of the chamber. ELISAs of TGF-alpha were measured at intervals during a 96-hour period. RESULTS: Aliquots taken during a 96-hour interval demonstrated passage of TGF-alpha in concentrations sufficient to stimulate hair cell regrowth. CONCLUSIONS: The apparatus allows study of RWM permeability to other substances and provides a basic model for study of RWM physiology. TGF-alpha is able to pass through a mammalian RWM.

Response of alopecia areata to DNCB: influence of auto-antibodies and route of sensitization.

The effects of treatment with topical 2,4-dinitrochlorobenzene (DNCB) were observed in fifty-one patients with alopecia areata of at least 9 months duration. Patients were sensitized either by the application of 500 micrograms of DNCB in acetone to the forearm, or by painting affected areas of the left side of the scalp with a 1% solution. A mild to moderate dermatitis was maintained by weekly applications of DNCB. When the sensitizing dose was applied directly to the scalp, significantly more patients showed poor reactivity as judged by the eczematous response obtained, although the two routes of sensitization had comparable effects upon regrowth of hair. Hair regrew significantly more frequently in females. The likelihood of regrowth was reduced in patients with hair loss of long duration and in those with immunological disturbances such as autoantibodies or low T lymphocyte numbers and responses. The relationship of these factors to the disease and to the response to DNCB treatment is discussed.
