Latanoprost, a selective FP prostanoid receptor agonist used in the treatment of glaucoma, has a hypertrichotic side effect. Using the macaque model of androgenetic alopecia, we examined the effect of latanoprost on hair growth. Eight monkeys were divided into 2 groups; one group received a daily topical application of 50 microg/ml of latanoprost for 5 months; a control group had a daily application of vehicle. For an additional 3 months, 2 monkeys from each group were given 500 microg/ml latanoprost, while the remaining monkeys continued with the previous treatment. Hair growth was evaluated by monthly photographs and phototricho-graphic analysis. Fifty microg/ml of latanoprost caused minimal hair growth. Latanoprost at 500 microg/ml induced moderate to marked hair regrowth with 5-10% conversion of vellus hairs to intermediary or terminal hairs. The vehicle group showed no effect. Further evaluation of latanoprost as an agent for treatment of human androgenetic alopecia is indicated.

Quantification of scalp hair--a computer-aided methodology.

A method for obtaining a quantitative assessment of hair density is described. First, a photographic image of the scalp is digitized onto a high-resolution computer graphics screen. Second, the frequency of each of 256 gray levels (one for each of 500 vertical X 500 horizontal = 250,000 locations on the screen) is obtained and the frequency histogram of gray levels is displayed. Third, a statistical procedure, gaussian mixture analysis, is used to resolve the frequency distribution into two normally distributed component distributions. The first component distribution describes the range of gray levels that are typically associated with hair. The second component distribution describes shades of gray that are typically associated with scalp. The statistical model provides a precise measure of the proportion of the head that exhibits gray levels in each of the two component distributions (hair or scalp). The proportion of the first component distribution is a scale-independent measure of hair density. The difference in this quantity before and after treatment provides an accurate quantitative determination of the change in hair density and hence of the efficacy of treatment.

The Mercedes incision in hair restoration.

Standard technique for hair transplantation includes plug composite grafts placed in circular recipient defects of smaller diameter. The plugs can also be placed in slit incisions that are temporarily dilated to accommodate grafts. Two drawbacks of these techniques are the appearance of row cropping resulting from the regular pattern and compression of individual grafts caused by scar contracture. These drawbacks result in tufting and a "doll's head" appearance. The expansile Mercedes incision as described here camouflages these failings. Donor minigrafts are taken with a biopsy punch knife or as strips in a standard fashion. A Mercedes logo-shaped defect is then made at the recipient site using a no. 11 scalpel to create a three-armed stellate incision. The incisions are rapidly made with the axis oriented parallel to the direction of the native hair shafts. The graft is then pushed in flush with the skin. The triangulation eliminates compression of grafts because they are spread in three directions rather than squeezing in a tight circular or slit configuration. The use of the stellate incision along with altering its rotational orientation in a haphazard pattern also prevents row cropping. Thus, the Mercedes incision is quicker and yields an overall improved result.
