Single-agent paclitaxel as first-line treatment of metastatic breast cancer: the British experience.

The efficacy and safety of paclitaxel (Taxol; Bristol-Myers Squibb Company, Princeton, NJ) was studied as first-line treatment for 30 consecutive patients with metastatic breast cancer. All patients had received adjuvant/neoadjuvant chemotherapy, 22 had prior hormonal therapy, and 26 had received previous adjuvant radiotherapy. Paclitaxel 225 mg/m2 was given as a 3-hour infusion without colony-stimulating factor support at 3-week intervals without excessive dose reduction or delays. The most common nonhematologic toxicities were alopecia (grade 3 in 29 patients) and peripheral neuritis (grade 2 or 3 in 14 patients). The objective response rate was 60% and responses were seen in all disease sites. The median duration of overall response was 30 weeks (range, 15 to 75+ weeks) and the estimated median survival time for all patients was 56 weeks (range, 1 to 82+ weeks). The response rate and survival times seen with single-agent paclitaxel are encouraging. Future studies must explore paclitaxel in combination with other regimens, with the hope that results will improve further.

Although the onset of alopecia areata has often been anecdotally linked with emotional stress, findings from the few controlled studies have not been univocal. The authors compared outpatients experiencing a recent onset of alopecia areata (N=21) with outpatients affected by skin conditions commonly believed as having a low psychosomatic component (N=102). Participants were administered Paykel's Interview for Recent Life Events, the Experiences in Close Relationships scale, the 20-item Toronto Alexithymia Scale, and the Multidimensional Scale of Perceived Social Support. Multiple logistic regression was used to control for age and gender. The total number of recent life events (last 12 months) was not different between the alopecia patients and the comparison subjects. Also, the alopecia patients and the comparison subjects did not differ in terms of the number of undesirable or major events. The comparison subjects even experienced a greater number of uncontrollable events. Alopecia areata tended to be associated with high avoidance in attachment relationships, high alexithymic characteristics, and poor social support. The results suggest that personality characteristics might modulate individual susceptibility to alopecia areata.

Carboplatin/etoposide as first-line chemotherapy in advanced ovarian carcinoma: a pilot study.

In a pilot study, 18 patients with advanced ovarian cancer were evaluated for tolerance and response to a combination treatment with a fixed dose of carboplatin (350 mg/m2 given i.v. on day 1) and escalated doses of etoposide (70-130 mg/m2 daily given i.v. on days 1-3) as first-line chemotherapy. The maximum tolerated dose of etoposide was 130 mg/m2 when given i.v. on days 1-3 in combination with 350 mg/m2 carboplatin given i.v. every 4 weeks. At these dose levels, bone marrow toxicity was manageable and did not appear to be cumulative. In all, 12 objective responses, including 9 complete responses (CRs) and 3 partial responses (PRs), were achieved in 18 patients; 6 of the 9 CRs were confirmed as pathological CRs by second-look surgery.
