Psychosocial and developmental factors in failure to thrive: one- to three- year follow-up.

Fifteen infants and children with a diagnosis of failure to thrive (FTT) were followed at one to three years postdischarge. Data were gathered from in-home interviews, psychosocial questionnaires, measurement of present height and weight, and administration of the Developmental Profile. Nine of the 15 children were still failing to thrive; the other 6 were considered recovered. The nonthriving group consisted of three subgroups: (1) developmentally retarded, (2) organic basis FTT, and (3) psychosocial FTT. The developmentally retarded children were significantly different in that they required more attention, had greater constipation, and engaged in less tantrum behavior. Further, they were significantly more impaired in the physical, self-help, and social areas of development. The difficulties in diagnosis and treatment of FTT children are discussed. Suggestions for the enhancement of medical management of this complex and life-threatening disorder are offered.

Abnormalities of the colonic regulatory peptides in Hirschsprung's disease.

We describe here a depletion of peptide containing nerves and cells in Hirschsprung's disease, in comparison with specimens of bowel taken from age-matched neonates with no evidence of chronic constipation. VIP content in the diseased specimens was reduced by almost 80%, from 110/+-10.6 (mean +/- SEM) pmol VIP/g wet weight of tissue in controls to 23.8 +/- 3.5 pmol/g in the mid-portion of the diseased specimens. In addition, the numbers of enteroglucagon and somatostatin cells in the mucosa were significantly reduced in the aganglionic portions. Enteroglucagon cells were reduced from 55 +/- 7 in controls to 27 +/- 2 in proximal portions rising to 44 +/- 3 and 49 +/- 4 cells/mm2 in middle and distal areas. Somatostatin cell numbers also fell, from 5.5 +/- 1.9 to 1.8 +/- 0.8, 2.5 +/- 0.7 and 3.8 +/- 0.9 cells/mm2 in similar areas. Further investigation of the abnormalities of the diffuse neuroendocrine system in Hirschsprung's disease may help in understanding the nature of this condition and provide additional information on the role of these peptides in the control of gut function.

Tropical paediatric intussusception--is it a different disease entity?

The presentation of intussusception in children in Zaria, Nigeria has been studied with the aim of identifying any features that may differentiate it from that which obtains in temperate countries. Our data reveal no difference in age of peak incidence, sex ratio, absence of preceding upper respiratory infection or lack of significant seasonal variation. However, there is an unusually high incidence (41%) of caecocolic intussusception. It is believed that this may account for other observed features, such as the low occurrence of constipation and other evidence of intestinal obstruction, and the relatively high incidence of "painless" intussusception and sub-acute or chronic intussusception. Twenty-nine per cent of the patients had only one or two of the usual diagnostic features of the disease and this contributed to delayed diagnosis. In all but two patients the condition was idiopathic and there was no evidence to implicate bacterial enteritis or parasitic infestation in the aetiology of the disease.
