[Chronic functional constipation in children: diagnosis and prevalence in a low-income community]

OBJECTIVE: To investigate the prevalence of chronic functionalconstipation in children aged under 11 years of a low-incomecommunity in Recife, Northeast Brazil and to assess the validity ofcomplaints of bowel habits for the diagnosis of constipation. METHODS: A community-based survey identified 536 eligiblechildren. Data collection was home-based through a structuredquestionnaire applied to the mothers. The definition of chronicfunctional constipation was based on painful defecation or witheffort, with stools of hard consistency and/or frequency of defecationless than three times per week, during at least the last thirty days. RESULTS: The prevalence of chronic functional constipation was17.5%. Straining was the complaint which obtained the highestsensitivity for the diagnosis of constipation (96.8%) while pain todefecate obtained the highest specificity (99.5%). CONCLUSIONS: Chronic functional constipation was a prevalentsymptom in this community, pointing out to the need of an activesearch in paediatric consultation. Straining was identified as thesingle complaint which better diagnosed constipation.

Long-term efficacy of polyethylene glycol 3350 for the treatment of chronic constipation in children with and without encopresis.

Seventy-four children (43 with chronic constipation, 31 with constipation and encopresis) treated with polyethylene glycol 3350 (PEG) for longer than 3 months were studied to assess long-term efficacy. The mean duration of PEG therapy was 8.4 months (range, 3-30). Weekly stool frequency, stool consistency, and symptoms associated with constipation improved significantly with PEG therapy in all 74 patients. In 31 children with encopresis, soiling ceased completely in 16 patients and frequency of soiling decreased significantly in all others. The average effective long-term dose of PEG was 0.7 g/kg/day. Long-term PEG therapy is effective for the treatment of chronic constipation with and without encopresis in children.

This paper reported the results of clinical observation on a treatment with Semen Persical decoction for purgation with addition (SPDPA) in type II diabetes mellitus. The effective rate of SPDPA on 106 cases of noninsulin dependent diabetes mellitus (NIDDM) was 79%. The efficiency of SPDPA was equivalent to glyburide. From the experimental study, it can be concluded that SPDPA could reduce blood sugar and relieve symptom in diabetic patients and rats. Its mechanism may be due to improving secretion of insulin, inhibiting production of glucagon, repairing insular endocrine cell, increasing endocrine pellet of insular B cell and improving composition of hepatic glycogen. In traditional Chinese medicine theory, the mechanism of therapeutic action of SPDPA in diabetes mellitus is based on synergistic regulation of benefiting Qi and nourishing Yin, activating blood circulation to dissipate blood stasis and loosening the bowel to relieve constipation.
