Comparison of staphylococcal and nonstaphylococcal endocarditis in narcotic addicts.

In a 54-month retrospective review, we compared the clinical features of 26 narcotic addicts with staphylococcal endocarditis (group 1) and ten other addicts with nonstaphylococcal endocarditis (group 2). The admission temperature and the respiratory rate of patients in group 1 were significantly higher (P less thn .05 and less than .02 respectively) than those of patients in group 2. Group 1 also differed from group 2 in the following variables: (1) bilateral multiple pulmonary infiltrates in 46% vs none in group 2; (2) greater incidence of symptoms referable to the central nervous system (50% vs none in group 2); and (3) gastrointestinal symptoms of vomiting, diarrhea, or constipation in 62% vs 10% in group 2. Serious cardiovascular, renal, and other complications were more frequent in staphylococcal endocarditis. Tricuspid regurgitation occurred with equal frequency in both groups and was of no value in differentiating staphylococcal from nonstaphylococcal endocarditis.

Following three years of the use of the tupla-clip we were able to send questionnaires to 115 women. The mean age was 33 years with 2--3 children. 57% of the tubal sterilizations were done for medical reasons, 7% for social reasons and 56% for personal reasons. 15% of the sterilizations were done concommitant to pregnancy. Among 243 tupla-clip sterilizations 2 pregnancies occured as early complications. The 115 women were asked regarding somatic complaints, psychic side effects, and social difficulties. The personal evaluation of this sterilization method was reported as good by 68%, doubtful by 26%, and negative by 6% of the patients. Technically this method of tubal sterilization is secure, fast, and the complication rate is low.
PIP: 115 women, mean age 32.63, underwent sterilization by application of Tupla-Clips. The mean pregnancy rate was 2.7 and the mean birthrate was 2.23. 57% of the sterilizations were performed under the medical indication. 85% of the sterilizations were interval sterilizations. There were 3 cases of hematoma, 16 cases of broken or dropped clips, and 2 pregnancies. 14% of the patients reported that their menstrual cycles were irregular after the operation. 12% of the women had abdominal pain either immediately or at some time after the operation. 15 patients reported an inflammation after the operation. 29% of the patients suffered from headaches and 17% from constipation. 2% of the patients were psychologically unsatisfied with the operation. 5 women reported that their sex lives got worse after the operation; 33 reported an improvement. 6% would not recommend the operation to others.

Management of late complications of Teflon sling repair for rectal prolapse.

Recurrent rectal prolapse or postoperative rectal stricture occurred in four of 88 patients (4.5 per cent) who underwent Teflon sling repair at the Lahey Clinic during the past 15 years. Management of these and six other similar patients referred for treatment suggests that young men appear to be at a higher risk for recurrence. Strictures may be more likely to develop in patients with a long history of prolapse or problems with constipation. Teflon sling repair followed by recurrent prolapse or stricture formation should probably be treated by low anterior resection.
