Lowered serum cholesterol following the ingestion of a hydrophilic colloid.

An attempt was made to test the possible clinical application of the cholesterol-lowering effect of hydrophilic colloids. 10 patients with high mean serum cholesterol levels during a 3-week control period and 9 with normal mean levels were each fed 11.5 g daily of undiluted hydrophilic colloid for 5 weeks. Serum cholesterol decreased significantly in 8 of the 10 (80%) patients with control-period hypercholesteremia and 6 of the 9 (66 2/3%) with control-period normal levels. These findings tend to confirm the observation that a simple and commonly used type of medication for chronic constipation, particularly in the elderly, may be a useful adjunct in the management of hypercholesterolemia.

Differential measurement of small and large bowel transit times in constipation and diarrhoea: A new approach.

Differential measurements of small and large bowel transit times were performed in 13 subjects iwth a radiotelemetering pressure-sensitive capsule incorporating less than 10mugCi of 51-Cr. Six patients had constipation. The other seven patients had diarrhoea due to the irritable bowel syndrome (3), following vagotomy and pyloroplasty (3), or due to laxative abuse (1). This new method enables the gastric, small intestinal, and colonic transit times to be measured differentially in the same subject. The capsule can be localized in the gut lumen by reference to the characteristic pressure pattern and in relation to bony landmarks by the radioactive marker as frequently as desired without recourse to radiographs. The results show that gastric emptying and small intestinal transit did not differ in constipation and diarrhoea. By contrast the mean colonic transit was significantly faster (P smaller than 0.01) in diarrhoea whatever the cause (17.5 plus or minus 4.1 hours) than in constipation (118 plus or minus 4.1 hours).

One particular form of an anorectal malformation--the anterior perineal anus--is frequently overlooked in the neonate period because defecation is easy at this age. However, with the change of diet, this malformation as a rule leads to chronic constipation. The lateral view at the beginning of an irrigoscopy usually shows a characteristic picture of a horizontal caudal limitation of the rectum, a dorsal pouch, a beakshaped fistula and an elevation of the anorectal transition. These findings contribute essentially to the clinical diagnosis and permit early operation.
