[Clinical picture and treatment of acute thallium poisoning]

The authors report results of treatment of 16 patients whose disease was caused by premeditated poisoning with thallium fluoride. The first signs of the disease appeared in 5 patients 30-38 hours and in the remaining 2-4 days after ingestion of poisoned food and were manifested by dyspepsia, leg pain. Then neurological symptoms of polyneuritis and encephalomyelitis prevailed. Within 1-3 weeks alopecia, xeroderma, herpetic phenomena appeared. Most patients showed disorders of the psycho-emotional sphere (intoxication encephalopathies of anxiety-paranoia type). Severe grades of thallitoxicosis were marked by involvement of the myocardium, liver and kidneys. Excretion of the nucleus is slow: with the urine (45%), with feces (55%). For correct diagnosis and for solution medical forensic problems examination of the content of thallium in the blood, urine, feces and hair is of importance. Phase II study with etoposide in previously untreated advanced breast cancer.

A phase II study was carried out to evaluate the efficacy and safety of etoposide used as first-line chemotherapy for patients with advanced breast carcinoma. A total of 20 patients received 230 mg/m2 i.v. etoposide per day for 3 days (total, 690 mg/m2 per course) every 4 weeks. A total of 95 courses were given. Observed responses included 3 partial remissions (PR) and 14 cases of stable disease (NC). The median duration of response was 6 (PR) and 5.6 months (NC). Contrary to the severe hematological toxicity in heavily pretreated patients described in previous studies, no substantial problems were observed in this trial. No dose reduction was necessary, and only once did leukopenia lead to a 1-week delay in therapy. An increase in platelets up to a maximum of 685,000/mm3 was seen in all patients, particularly in those with bone metastases. No relation to the quality of remission or pretreatment was seen. Nausea, vomiting, and fatique were mild and transient, but alopecia occurred in all cases. One patient developed nonfatal anaphylactic shock after etoposide treatment.

Locations of moose in northwestern Canada with hair loss probably caused by the winter tick, Dermacentor albipictus (Acari: Ixodidae).

Five hundred two trappers representing 389 registered traplines in northern Alberta, northern British Columbia, Northwest Territories and Yukon Territory (Canada) responded to a questionnaire on the occurrence of hair loss and the winter tick (Dermacentor albipictus) on moose (Alces alces). Results suggested that winter ticks may occur as far as 62 degrees N. Several sightings of moose with presumed tick-induced hair loss near Kluane Lake, Yukon Territory, suggest the possibility of introduction of this serious pest into the moose population in Alaska.
