The protective role of the HLA-DR locus in patients with various clinical types of alopecia areata.

One of the genetic factors associated with the development of alopecia areata (AA) is the HLA locus. The study comprised 52 patients with AA aged 10 to 64 years. The frequences of HLA-DRB alleles in the patients and controls were compared. The control group comprised 152 healthy persons. Familial occurrence of AA was seen in 7 (13.5%) cases. A significantly lower frequency of HLA-DRB1*03 was observed in patients with AA comparison with the control group. In all patients with AA, alleles HLA-DRB1*15/*16 occurred more frequently than in the control group, but this was not significant after correction. In the group of patients with more severe forms of alopecia areata (alopecia areata totalis/alopecia areata universalis) there was no significant difference in HLA-DR allele distribution.

PURPOSES: To ascertain possible evidence for radiosensitive subgroups in the Japanese atomic-bomb survivors. MATERIALS AND METHODS: Time- and age-adjusted relative risk models were fitted to the Japanese atomic-bomb survivor mortality data, taking account of the modifying effects of acute injury status and adjusting for random dosimetric errors. RESULTS: For leukaemia, there were statistically significantly higher relative risks among those persons reporting either epilation (two-sided p=0.03) or burns (two-sided p=0.02), but after adjustment is made for the effects of dosimetric error these results become statistically non-significant (two-sided p>0.05) For all other endpoints (cancers other than leukaemia, benign neoplasms, cardiovascular disease, non-cancer and non-cardiovascular disease) there were no statistically significant modifying effects on relative risk of acute injury status, whether or not adjustment is made for the effects of dosimetric error. CONCLUSIONS: Apart from leukaemia, there are generally no indications for cancer or other disease endpoints of variations in dose-response by acute injury status. The increased risks observed for leukaemia among those reporting epilation or burns are much diminished after adjustment for dosimetric error, and so there is no strong evidence of a sensitive subgroup within the Life Span Study cohort.

PURPOSE: To evaluate the effectiveness of stereotactic fractionated radiotherapy (SFRT) in the treatment of optic nerve sheath meningioma (ONSM). METHODS AND MARERIALS: Between 1994 and 2000, a total of 39 patients with either primary (n = 15) or secondary (n = 24) ONSM were treated with SFRT and received a median total tumor dose of 54 Gy using 1.8 Gy/fraction. RESULTS: The radiographic response to SFRT was documented in all patients as stable disease (no change) except for 1 patient with a partial response. After a median follow-up of 35.5 months, all patients with ONSM were alive without recurrence. The visual fields and visual acuity were improved in 6 of 15 and 1 of 16 examined eyes in patients with primary ONSM, respectively, and in 6 of 24 and 7 of 26 examined eyes in patients with secondary ONSM, respectively. Stable visual fields and visual acuity was observed in 8 of 14 and 15 of 16 patients with primary ONSM, respectively, and in 17 of 24 and 19 of 26 patients with secondary ONSM, respectively. Except for reversible alopecia and erythema, no other SFRT-related toxicity was observed. CONCLUSION: SFRT represents a very effective and low-toxic treatment modality for ONSM. Despite a median follow-up of 3 years, this series of primary ONSM holds promise for future studies. It adds substantial evidence that SFRT may definitely become a standard treatment approach in selected cases of ONSM.
