What is AIDS?

PIP: Acquired immunodeficiency syndrome (AIDS) is caused by the human immunodeficiency virus (HIV). According to the World Health Organization, 4 million people have developed AIDS since the beginning of the pandemic; by mid-1994, more than 16 million adults and over 1 million infants had been infected with HIV. The vast majority of those infected with HIV will develop AIDS; the average time between the two events is 10 years. For children and those with poor nutrition, the time is shorter. There is no cure for AIDS, and almost all of those with AIDS will die of it. Development of a vaccine has been unsuccessful. Since HIV destroys cells in the body's immune system, the infected person can easily develop tuberculosis, meningitis, and chronic diarrhea. Although an infected person may have no symptoms in the early stages of the disease, he or she can still transmit the disease to others. A person can be infected with HIV through contact with infected blood, semen, and cervical or vaginal fluids; through sexual intercourse; through transfusion of blood or blood products infected with HIV; through injection or piercing of the skin with an instrument contaminated with HIV; or from an HIV-positive mother to her child during pregnancy, birth, or breastfeeding. HIV cannot be transmitted by coughing or sneezing; handshakes; insect bites; contacts at work or school; touching or hugging; using toilets; water or food; using a telephone; kissing; swimming pools; or sharing cups, plates, or other eating and drinking utensils.

WHO calls on South African officials to speak out.

PIP: World Health Organization (WHO) officials have suggested that President Nelson Mandela should declare a national health emergency with regard to the tuberculosis epidemic in South Africa. In July 1996, WHO announced that South Africa had about 350 cases of tuberculosis per 100,000 population, the worst in the world. The incidence of multi-drug resistance is also rising. In an article published in the New York Times on October 13, 1996, unnamed WHO officials were quoted as saying that President Mandela and Archbishop Desmond Tutu should speak about their experiences with the disease in order to increase public awareness. The article stated that a rise in drug use and the spread of human immunodeficiency virus (HIV), both of which depress the immune system and create vulnerability to tuberculosis, had occurred in South Africa with the renewal of international contacts after the lifting of sanctions imposed on the apartheid government. The South African health system currently spends about 20% of its tuberculosis budget on hospitalizing the 2% of patients with drug-resistant tuberculosis. Experts and WHO officials question the government's commitment; health administrators new to their jobs, tight budgets, and a calculated shift of power from Pretoria to the provinces are permitting the disease to go unchecked.

China urges caution in herbal cures for AIDS.

PIP: Kang Kaiyi from the Shanghai Municipal AIDS Surveillance Center is quoted as saying that global AIDS control may benefit from China's rigorous and scientific research on the impact of traditional herbal medicines on AIDS treatment. Media attention to major breakthroughs in research should be cautious. Sufficient evidence about effectiveness should be compiled by pharmacologists before making announcements to the world press. China now has 133 advanced AIDS cases. The number of HIV cases increased from 4305 in August 1996 to 5157 at the end of October 1996. 86 people have already died of AIDS. Kang Kaiyi and others in the research group found that a locally-developed herbal medicine was partially effective in improving the immune systems of animals. There were no long- or short-term toxic effects. The herbal medicine was administered to several AIDS patients, who showed a reduction in symptoms and resumed functioning of their immune systems. The tests showed that HIV RNA was evident in the blood of these AIDS patients. Kang indicated that the Chinese press should not overemphasize these preliminary findings. Further evidence is needed before a major pronouncement can be made to the scientific community. There is need to control medicine quality and to perform detailed observations in a clinical setting. The objective is to develop effective, affordable drugs for the treatment of AIDS.

Superantigens are potent activators of the immune system, causing a variety of diseases, ranging from food poisoning to septic shock. Here, we examined the effects of different toxic shock syndrome toxin 1 (TSST-1) concentrations on the activation, proliferation and synthesis of interferon-gamma (IFN-gamma) and interleukin-4 (IL-4) in purified naive human CD4+ T cells in a serum-free in vitro system. TSST-1 given in low doses (1-10 pg/ml) generates a pronounced T helper 2 (Th2)-like cytokine profile, characterized by elevated IL-4-expressing T-cell populations and reduced IFN-gamma-producing populations, whereas higher doses (100 pg/ml) induce a Th1-like profile, with increased expression of IFN-gamma and reduced expression of IL-4. These patterns were even more pronounced by adding exogenous cytokines like IL-12 and IL-4 and by the type of antigen-presenting cells (APCs). Thus, B cells induced Th2 shifts, whereas monocytes favoured Th1 induction. Moreover, IL-12 in conditions with B cells counteracted their Th2 bias. Interestingly, in purified naive T-cell cultures, containing a small population of HLA-DR+ T cells, Th1/Th2 differentiation can be induced by TSST-1 too. There, Th-cell polarization is strongly dependent on TSST-1 concentration, indicating that this is a key parameter in regulating the differentiation of T cells. In conclusion, our data show that Th1/Th2 differentiation of TSST-1-stimulated naive T cells is controlled by the type of APCs, and in APC-depleted cultures, it depends on the presence of HLA-DR+ cells and TSST-1 concentration.

Plant vaccines: edible, but how credible?

PIP: Researchers are developing vaccines which will be produced by edible plants, then consumed by human populations in need of such vaccine. The plants are genetically engineered to carry genes from disease-causing microbes. Within those microbes, the genes control production of antigens which provoke immune responses in people infected with the microbes. Once inside of a plant's DNA, the genes force the plant to produce the desired vaccinating antigens. While this is far from the first time that foreign genes have been successful spliced into plant DNA, this is the first time that recombinant biotechnology is being used to make plant vaccines for widespread use among humans. Such edible plant vaccines will be less expensive to produce compared to the current production process using microbial fermenters, they can be fitted with a very large number of foreign genes, they can be administered orally, and animal models suggest that they should be able to stimulate the more general antibody and cellular immunity needed for protection against most disease-causing microbes as well as the local immune system of the gut. Plants ideally suited to be vaccine producers and vehicles are tasty, cheap, and can be eaten raw, since heat would inactivate protein antigens. It has recently been shown that foreign genes can be inserted into banana DNA.
